You will at least need a deed granting your property into the trust and a trust certification.

Does anyone know besides the land trust document itself, what other documents or forms must be filled out when forming a land trust? Any help would be greatly appreciated.

Sounds like you have not done a trust before.
My advise is get an attorney to set this up because if done incorrectly you are in real trouble.
Attorneys will charge $350 - $700 to do this for you.
Another option is get Bill Bronchick’s “Step to Step Guide to Land Trusts” it will teach you how to do this yourself. It is state specfic and has all forms needed. Once you have this you can reuse over and over at no charge, compared to attorneys.
