You can get an example of an option form from the American Note Network website. They call it a commitment letter and its listed under one of their articles titled something like “notebrokers toolkit”. You may have to tweak it a bit, but it should work fine.
As far as the assignment form I see them all the time down at the courthouse when banks transfer(assign) loans among each other. They are typically one-page, one-paragraph documents which should be easy to dupicate for your situation.
