I am selling a property private sale, the sales agreement has an as is clause. do I still need to attach a separate disclosure form telling that I put a new roof on, and also do I need a separate lead disclosure?

I would think that a new roof would be a bonus to help motivate the buyer to close. Esp. if thew warranty transfers. Even if its not required, this would be a good thing to pass on.

Required disclosures vary from state to state, but I can tell you that if the property was built prior to 1978, you must give them the LBP pamphlet and disclosure. Additional disclosures are probably required beyond that in your state.
