Jan,
A note is an obligation you agree to honor. The mortgage is public evidence of that obligation. What date along the line the mortgage is recorded determines which is senior, and which are junior to it. It does not say on either the note or mortgage “1st mortgage” “2nd mortgage” etc. Just use whatever generic form works best in your state.

These are links to the Fannie Mae site where you will find Mtg docs and Notes, which are state specific, as well as some general docs too. You can download and modify these as your template.
