I don’t think what you’re describing will work because that is not your mother’s name anymore. The ID you describe will not satisfy a notary (at least not in California). The crux of the problem is having some identification to show a notary that “Ann Doe” and “Jane Ann Smith” are one and the same. I am nearly one hundred percent positive that my mother does not have a picture identification with her maiden name on it.

Is there anything illeagal about simply making a picture id with my mother’s middle and maiden name on it? Would a home made id satisfy notary or can you only use a drivers license?

It would be easy enough to make. I would simply take a picture of her and glue it to a blank business. Then next type something like “Ann Doe-- Certified Trust Officer” on it along with her address. Then go get it laminated.

Any help is appreciated.
